Medical Passport (2 pages)

What is the Medical Passport? It is a document that serves two purposes: patient documentation of medical diagnosis and treatment and a tool of communication for all health providers. This medical passport is given to all of our Transition Program patients as a portable medical record. It is popular among the teenagers and can be customized to your institution.

We offer this tool in a Publisher format. It is a 2- page document. On the first side, patient information and digital picture can be added. On the other side, medications, diagnosis, allergies and contact numbers can be added. We print this as a credit card-size document and laminate it. We mail these to the patient when updates are required.

What is the Transition ID? It is a unique identifier independent of patients' medical record. We use this number to track adherance with the use of the Medical Passport. We call the adolescent/ young adult every 3-6 months to confirm that he or she is carrying this identification, which we accomplish by asking for the last 4 digits of the transition ID number. Transition Scale Assessment

A health-provider administered survey that takes no more than 10 minutes to complete. It is designed to help providers identify which area(s) of transition require education and also to track each patient’s progress.

Transition Readiness Survey

A self-administered instrument that takes 10 minutes to complete. It is designed to help track and determine the transition status of patients in regards to their readiness to move from pediatric to adult care.
