The UNC Transition interdisciplinary team is composed of enthusiastic and dedicated members whose goal is to improve patients' health outcomes with evidenced based information. The members of this team include patients, coordinators, nurses, social workers, educators, psychologists, pharmacists, public health scientists, recreational therapists, adolescent specialists, students of several disciplines, dieticians, administrators, computer scientists and physicians. We have established collaborations that go beyond the walls of our institution and welcome interested researchers to join our team.
The #1 team member in our group is the adolescent. This program has been built with the input of all the adolescents and their families who participate.
